Blog:Enrolling in SC Choices
We've told you how to choose a health plan. Now, we want to make sure you can enroll in SC Choices easily and properly!
First, you will need to gather some personal information before you enroll, including:
- Your full name
- Medicaid ID number
- Date of birth
- Address and telephone number
- Your doctor (or clinic) if you have chosen one
- Doctor's address
Some of these are part of your protected health information, so don't give the information to anyone other than SC Choices staff or your doctor or clinic.
There are four easy ways to enroll:
- Phone: To enroll by phone, call our call center.
- Mail or fax: To enroll by mail, watch for the enrollment packet. We will mail it to you once we know that you qualify for Medicaid. Open it, read it, then fill out and mail the enrollment form!
- Internet: Enroll on our website. Start by choosing Log in on the left menu.
- In person: Go to a community meeting, where a trained enrollment counselor can help you enroll. Some enrollment counselors speak Spanish. To find a community meeting, call our call center or check out our calendar of events.
