South Carolina Healthy Connections Choices is a state program that helps people in Medicaid enroll in (join) health plans to get Medicaid services.
On this site you can:
- Choose a health plan and enroll
- Search for doctors, clinics or hospitals near you
- Find a meeting near you where you can enroll in person
- Find out how to call, email or send a fax to an enrollment counselor
